Solution
Issue: the Chat button icon doesn't appear, but the Admin login works and the SightMaxWebService.svc file can be browsed. The following error can be seen in the Event Viewer.

Exception message: The pipe name could not be obtained for net.pipe://localhost/service/VisitorChat.
Solution:
The issue is due to ASP.NET impersonation being enabled in the asp.net root web.config file. Removing the following entry resolved the issue with the SightMax application.
Edit C:\WINDOWS\Microsoft.NET\Framework\v2.0.50727\CONFIG\
web.config
and remove
<identity impersonate="true" userName="" password="" />
